DOCUMENTS REQUIRED FOR SUBMITTING AN APPLICATION:
1. High school diploma with transcript of grades – which qualifies the candidate to undertake higher education studies at all levels in the country of issue.
2. A sworn translation of the high school diploma and transcript of grades to Polish – if the original documents are in English, you can submit the sworn translation to Polish after getting the decision of conditional acceptance.
3. English language certificate – list of accepted certificates HERE.
Candidates exempt from having to provide an English language certificate:
– IB or EB Diploma holders,
– candidates with English as the language of instruction during high school,
– candidates with Polish matura exam who passed advanced English with a minimum grade of 80%,
4. Passport or personal ID – personal IDs can be used only by candidates from EU countries, in other cases candidates should use passports. In case of having a Polish passport/ID and passport/ID from a different country, the candidate should use Polish passport/ID.
5. Application Form – generated by the IRK system after the candidate enters all required data. It can be found in the „print outs” tab. The candidate must print the form, sign it with their first name and surname along with the dates in the two required places on the last page, and then upload it to the IRK system.

Due to the introduction of new legal regulations in Poland, effective from July 1, candidates holding high school diplomas issued in countries outside the EU, OECD, EFTA, Ukraine, and China (applies only to the Gaozhong certificate) will be required to submit, during the application process, one of the following:

In addition, candidates who submitted scans of all the required documents in the online registration system during the application process will also be required to sit a subject-specific entrance exam, which will be conducted online. Only after successfully passing this exam will their application be considered by the Admission Board.

DOCUMENTS REQUIRED AFTER THE DECISION OF CONDIDTIONAL ACCEPTANCE:
(Documents must be original versions of those submitted during the application process and must be delivered to the Admissions Office within the specified deadline.)
1. High school diploma with the transcript of grades, either legalized in a Polish Embassy or Consulate or provided with an Apostille, depending on the country where the diploma was issued.
Apostille:
You can see the list of countries which signed the Apostille Convention HERE, and you can check the list of competent authorities/offices responsible for the Apostilles HERE.
If your diploma was issued in one of the countries on the list, it will need to have an Apostille attached to it. Correct Apostille should be issued for the original diploma and transcript and confirm the signature of the person signing the diploma and transcript (usually school principal or other school authority), not the signature of a notary, translator, or another official.
Both the diploma and the transcript should have an Apostille. If your grades from the subjects we require are on different certificates, both of those certificates need an Apostille.
The documents should be translated after the Apostille is issued. The translator’s signature does not need to be confirmed by another Apostille.
Legalization:

The documents that should be legalized are the original documents, not copies or translations.
2. Translation of the high school diploma and transcript of grades to Polish – done by a sworn translator. An Apostille should be translated to Polish as well.
3. English language certificate.

5. Proof of vaccination against hepatitis B – translated to Polish or English.
6. Eligibility certificate – if applicable.
7. Notarized parental/guardian consent form – if applicable.
8. An administrative decision from the Polish school superintendent confirming recognition of the certificate, or a written statement issued by the Director of NAWA confirming that the certificate entitles the holder to apply for admission to higher education in Poland– if applicable.

Please note that if a certificate or other document entitles to apply for admission only to specific higher education programs in the state of issuance, it shall be recognized as a document giving a right to apply for admission to such higher education programs in Poland which are identical or similar in terms of curriculum.
